Consent for Telemedicine and Telehealth Services
This consent describes the nature, benefits, risks, alternatives, privacy considerations, and limitations of telemedicine/telehealth services that may be provided by NATT Weight Loss and Primary Care Services. Texas law requires informed consent for telemedicine or telehealth services in applicable circumstances.
Nature of telehealth
Telehealth uses electronic communications to allow a healthcare professional and patient to communicate when they are not physically in the same location. Depending on the service, this may include video, audio, secure messaging, or other legally permitted technologies.
Potential benefits
- Convenient access to appropriate care and follow-up.
- Reduced travel.
- Improved continuity for services suitable for remote delivery.
Potential risks and limitations
- Some physical examinations cannot be performed remotely.
- Technology, internet, audio, or video failures may interrupt the visit.
- Information available remotely may be incomplete.
- Electronic communication has privacy and security risks despite reasonable safeguards.
- The provider may determine that an in-person examination, laboratory testing, imaging, urgent care, emergency evaluation, or specialist referral is necessary.
Patient identity and location
You may be asked to verify your identity and physical location at the time of a telehealth encounter. Your location may affect whether the provider may legally and appropriately provide the service.
Privacy and confidentiality
NATT will use reasonable measures designed to protect the confidentiality and security of telehealth communications and electronic health information. Telehealth records are handled in accordance with applicable healthcare privacy and recordkeeping requirements.
Prescriptions
A telehealth visit does not guarantee that any medication will be prescribed. Prescribing decisions remain subject to applicable federal and Texas law, professional standards, and the provider’s clinical judgment.
Technical failures
If a connection fails, NATT may attempt to reconnect, contact you by another approved method, reschedule the appointment, or direct you to another level of care.
Emergencies
Telehealth is not a substitute for emergency medical care. Call 911 or seek emergency treatment for severe or potentially life-threatening symptoms.
Alternatives and voluntary participation
Alternatives may include an in-person visit, another qualified provider, urgent care, emergency treatment, or other appropriate services. You may ask questions before consenting. You may withdraw consent to future telehealth services, subject to applicable law and the effect of services already provided.
Consent documentation
By providing consent through NATT’s approved workflow, you acknowledge that you understand the nature, potential benefits, risks, alternatives, and privacy considerations of telehealth and voluntarily agree to receive telehealth services when the provider determines they are appropriate. Consent should be documented in the patient record as required by applicable law.
