NATT Weight Loss and Primary Care Services

First-Time Patients

What to prepare before your first NATT appointment

Welcome to NATT. A little preparation can help make your first appointment more useful and efficient.

What to Have Ready

Identification

Bring a photo ID and current insurance information when applicable.

Medication List

Include prescriptions, over-the-counter medications, vitamins and supplements.

Relevant Records

Bring recent laboratory results, imaging reports or other records related to the reason for your visit when available.

Your Questions

Write down the main concerns and goals you want to discuss with Alison.

For Medical Weight-Loss Visits

Be ready to discuss your weight history, previous weight-loss approaches, nutrition and activity patterns, current medications, health conditions and goals. A consultation does not guarantee a prescription. Treatment is based on an appropriate medical evaluation.

For Primary-Care Visits

Bring information about your current symptoms, diagnoses, medications, allergies, family history and any recent test results that may be relevant.

Protect your privacy: Do not send medical history, diagnoses, medication lists or other private health information through ordinary website email. Use the clinic’s approved patient or scheduling system when clinical information is requested.
