Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, HOW YOU CAN GET ACCESS TO THIS INFORMATION, AND CERTAIN RIGHTS YOU HAVE. PLEASE REVIEW IT CAREFULLY.
Our legal duties
When NATT is subject to HIPAA, we are required to maintain the privacy and security of PHI, provide this Notice, follow the Notice currently in effect, and notify affected individuals when required following a breach of unsecured PHI. NATT also complies with applicable Texas medical-privacy requirements. If NATT creates or receives records protected by 42 CFR Part 2, those records are handled in accordance with applicable Part 2 protections in addition to other applicable law.
How we may use and disclose health information
Treatment
We may use or disclose health information to provide, coordinate, or manage healthcare and related services, including sharing appropriate information with healthcare professionals involved in treatment.
Payment
We may use or disclose health information for lawful billing, eligibility, claims, reimbursement, collections, and other payment activities.
Healthcare operations
We may use or disclose information for lawful healthcare operations such as quality assessment, credentialing, auditing, compliance, training, business management, and administrative functions.
Appointment and administrative communications
We may contact you about appointment requests, confirmations, reminders, follow-up needs, or other administrative matters.
Persons involved in your care
When permitted by law and appropriate under the circumstances, limited information may be shared with a family member, personal representative, or another person involved in your care or payment for care.
Required by law and public-interest purposes
We may use or disclose PHI when required or permitted by applicable law, including certain public-health activities, health oversight, judicial or administrative proceedings, law-enforcement purposes, workers’ compensation, reporting duties, or to prevent a serious and imminent threat where legally permitted.
Business associates
We may use service providers that perform functions involving PHI. When HIPAA requires it, appropriate business-associate agreements and safeguards must be in place.
Uses and disclosures requiring authorization
Uses or disclosures not otherwise permitted by law generally require written authorization. You may revoke an authorization in writing, except to the extent NATT has already acted in reliance on it. Special rules may apply to psychotherapy notes, certain marketing activities, sale of PHI, reproductive-health information, substance-use-disorder records, and other specially protected information.
